The Hidden Epidemic of High-Functioning Depression
At first glance, high-achievers look like theyโre thriving. Theyโre climbing the ladder, juggling roles, and getting things done. But behind many outwardly โsuccessfulโ lives is a quieter truth: millions are struggling with a hidden form of depression that doesnโt look like what weโve been taught to expect.
Dr. Josephโs own journey is striking. While leading groundbreaking clinical research, appearing on national TV, and serving on prestigious boards, she realizedโduring a talk for healthcare professionals during the pandemicโthat she was barely holding it together. That moment led to her latest book, High Functioning, and the first empirical research on this often-misunderstood condition.

โWeโre still using a broken model,โ she explains. โWe wait until people crash before offering help. But what about the ones silently breaking down while showing up every day?โ
This is a key distinction. Unlike classic clinical depression, high-functioning depression hides in plain sight. Sufferers may be hitting milestones while internally battling numbness, poor sleep, restlessness, and a loss of pleasureโa symptom known as anhedonia. As Dr. Joseph notes, โPeople say, โI feel meh,โ but donโt realize thatโs a red flag.โ
Introducing the 5 Vs Framework
In response, Dr. Joseph developed the 5 Vs Framework, an actionable roadmap for reclaiming your joy:
- Validation โ Acknowledge what you feel without judgment.
- Venting โ Express it through writing, talking, or movement.
- Values โ Reconnect with what truly matters to you.
- Vitals โ Protect your mind-body health: sleep, diet, movement, tech boundaries, and relationships.
- Vision โ Envision a future grounded in joy, not just achievement.
This isnโt about abandoning ambitionโitโs about leading from a place of presence rather than performance.

About Todayโs Guest, Dr. Judith Joseph

Judith Joseph, M.D., M.B.A., is a Board Certified Psychiatrist. Dr. Judith is Chair of The Women in Medicine Initiative for Columbia University Vagelos College of Physicians & Surgeons. Dr. Judith is a clinical assistant professor of Child and Adolescent Psychiatry at NYU Langone Medical Center. She has conducted several clinical research studies in pediatric, adult, geriatric, and womenโs mental health as the Principal Investigator of her research lab, Manhattan Behavioral Medicine.
Dr. Judith developed the T.I.E.S. method to address mental health symptoms in menopause. She is on the board of Letโs Talk Menopause, a national non-profit organization that recently advocated in December 2023 for the Menopause Research and Equity Act in the US Congress alongside Congresswoman Yvette Clarke. Dr. Judith is conducting the first of its kind research study in High Functioning Depression in her laboratories and will publish a book on High Functioning Depression with Little Brown Spark in 2025.
